Community Care

The Community Care Services Division provides a wide range of services to the elderly and disabled that help them preserve their dignity by maintaining their independence at home. These include:

• Primary Home Care / Family Care (PHC/FC)
• Community Based Alternatives (CBA)
• Emergency Response Services (ERS)

Outreach Health Services Provides these services through the Texas Department of Aging and Disability Services (TDADS) to individuals who meet income and Medicaid eligibility requirements under the CCAS (Community Care for the Aged and Disabled) and CBA (Community Based Alternatives) programs.  We also offer the services on a private pay basis for those who do not qualify through TDADS.

An optional program available to clients is the Consumer Directed Services or "CDS" program. CDS gives Medicaid beneficiaries in Texas greater control and responsibility for their own personal care. Click HERE  for a brochure for more information on the CDS program.
