Services
Assured Care provides services for ALTCS Medicaid members and AZ DDD members.
Assured Care is here to make thriving at home a reality. Our companionate care team provides the support and resources needed for DDD members, seniors, and those managing chronic health issues or needing extra care to live comfortably at home. Assured Care will work with each family and their agency Case Managers to create a care plan and schedule tailored to their needs and preferences.
Attendant Care Services (ATC)
ATC provides hands-on assistance for the functional needs of the member who has a physical disability, chronic health issue and/or just may need assistance with activities of daily living.
Respite Care Services (RSP)
Respite care provides short-term relief for primary caregivers. It can be arranged for just an afternoon or for several days or weeks.
Housekeeping Services (HSKG)
HSKG aids in the performance of routine household activities at the member’s place of residence.
Habilitation Services (HAH)
HAH service provides a variety of interventions designed to maximize the success of DDD members. Habilitation services are centered around DDD member’s habilitation goals to assist in keeping, learning, and/or improving skills and functioning for their daily living activities.
